I fully understand that F.E.S. Athletic staff members are not physicians nor are they medical practitioners of any kind. I hereby release the F.E.S. Athletic staff to render temporary first aid to my child in the event of any injury or illness, and if deemed necessary by the F.E.S. Athletic staff to contact our physician, and seek medical help, including transportation by an F.E.S. Athletic staff member or representative, whether paid or volunteer, to a healthcare facility or hospital of my preference or a paramedic contact.

I recognize my obligation to make my child and myself aware of the risks and hazards associated with the sporting activity they are enrolled in. I further recognize that my child may suffer injuries that are possibly minor, serious, or catastrophic in nature as recreational sports can be dangerous and may lead to potential injury. I agree to indemnify, protect, and hold harmless F.E.S. Athletic Association, its officials, employees, agents, and servants of any and all kinds from any and all claims, demands, actions, suits, damages, loss and expense of whatever kind or nature to any property arising out of this conjunction with this activity. I, the undersigned, hereby understand that insurance, which would cover my child in the event of any activity sponsored by Youth Basketball of America and Amateur Athletic Union.
